The first thing to do is take plenty of maps. Sure, you might have a GPS, but the size of the screen is usually a limitation. If you really want to get a good view of things, bring along a few paper maps. Make sure they are both of a big area, and of a detailed area of where you are. That way you'll always be able to check your plans from several different perspectives.
Taking some food is always a good idea. Sure, you'll be able to buy food wherever you are going, but having some snacks can help if you have to wait a while for a train or a bus. Some non perishable snacks can be great to give you a boost of energy right when you need it.
Water should be your best friend while traveling. Not only does water keep you from getting dehydrated, it can keep you from getting jet lagged as well. And if you are on vacation, chances are you are going to be doing a lot more walking than usual, so you'll need to drink more water than usual.
If you are going on a driving trip, then make sure your car is taken care of before you leave. Check the tire pressure, oil level, and all that good stuff. It might be a good idea to have the oil changed before you leave also.
Batteries are very important. You undoubtedly will be using a lot of hand held electronics, and it can be troublesome if they break down. That's why having plenty of extra batteries is very important when traveling.
A first aid kit is also essential. Sure, you're going to be close to hospitals and doctors (unless you are on some kind of safari) but having some band aids and some Advil handy is a good idea.
